Can you switch from weight loss injections to weight loss pills?

Whether you’re tired of needles, after something more convenient, or curious about new oral GLP-1s, you may be wondering whether you could swap your weekly injection for a daily pill. Most people can switch. But doing so safely involves more than simply stopping one medication and starting another.

Why might you switch from weight loss injections to tablets?

There are lots of reasons why someone taking GLP-1 weight loss injections might be thinking about switching to the new generation of weight loss tablets:

  • You're not getting along with injections, whether due to needle anxiety, injection-site reactions, or side effects that aren’t settling down.

  • You’ve run into practical challenges with injections, particularly if you travel often or find storing injection pens in the fridge inconvenient.

  • You've had great results but don't want to inject forever. A tablet may feel like a more sustainable long-term maintenance option that you can stick with.

  • You're worried about regaining weight if you stop treatment altogether. Continuing treatment with a tablet may feel preferable to stopping completely.

Whatever your reason, it's understandable to be curious. The arrival of oral GLP-1s is one of the biggest recent developments in weight loss medicine, and as more become available, many people using injections are likely to wonder whether switching could be for them.

Which weight loss tablets can you switch to?

Several oral GLP-1 medications are either available or expected to become available in the coming years. The most anticipated options include:

  • Wegovy tablet – a tablet version of Semaglutide, the same active ingredient used in Wegovy injections, taken once daily in the morning on an empty stomach.

  • Foundayo – a new once-daily oral GLP-1 medication called Orforglipron that does not require fasting instructions in the same way as oral Semaglutide.

How does switching from injections to tablets work?

Switching may be possible for many patients, but it's not usually as simple as replacing your weekly injection with a tablet the next day. In most cases, switching is likely to involve:

  1. Stopping your current injection at an agreed point in treatment

  2. Allowing an appropriate gap before starting the new medication

  3. Starting the tablet at a suitable dose

  4. Gradually increasing the dose over time if required

Even if you're taking a higher dose of an injection, you wouldn't normally start an oral GLP-1 treatment at its highest strength. This gradual dose escalation guide helps reduce the risk of side effects such as nausea, vomiting, diarrhoea, and stomach discomfort.

The exception is if you’re switching from the Wegovy injection to the Wegovy tablet. As both contain Semaglutide, most people can switch directly from the 2.4mg injection to the 25mg tablet without having to complete the dose escalation again.

The exact timing and dosing schedule will depend on the medications involved and your individual circumstances. Your prescriber will be able to recommend the safest approach.

Which injection-to-tablet switches are possible?

Most people can switch from any weight loss injection to any oral GLP-1 tablet. For example:

  • Mounjaro injections to the Wegovy tablet

  • Mounjaro injections to Foundayo

  • Wegovy injections to the Wegovy tablet

  • Wegovy injections to Foundayo

Are tablets actually more convenient than injections?

Many people assume tablets are more convenient than injections, but that isn't always the case.

Weight loss injections are taken once a week, meaning there's little day-to-day planning involved. For some people, this “do-and-forget" approach is easier than remembering to take a tablet every day.

On the other hand, tablets remove the need for needles, sharps disposal, and refrigerated storage, which can make them appealing if you travel frequently or dislike injections. But you must remember to take them at the same time every day for them to be effective.

If switching to the Wegovy pill, there are even more practical considerations. The Wegovy pill must be taken correctly (1):

  • First thing in the morning on an empty stomach

  • With a small amount of water (no more than 120ml)

  • At least 30 minutes before food, drink, or other medications.

Some people find this daily routine straightforward, while others find it hard to stick with it consistently.

Ultimately, convenience is personal. Some people prefer the familiarity of taking a daily tablet, while others find a weekly injection simpler and easier to remember.

Will tablets work as well as injections?

This again depends on which injection you’re currently on and which tablet you’re switching to.

  • If you’re switching from Mounjaro, you may find appetite suppression isn’t quite as strong and weight loss slows slightly. Mounjaro has produced the greatest weight loss in clinical trials among all weight loss medications, including newer GLP-1 tablets (2).

  • If you’re switching from Wegovy injections, you can expect weight loss to roughly stay the same, especially if you’re switching to the Wegovy pill. Clinical trials found that both oral and injectable Semaglutide produce broadly similar weight loss at comparable doses (3-4).

That said, results from clinical trials don’t tell the whole story, and the most effective treatment on paper isn't always the treatment that works best in real life. Some people find tablets easier to fit into their routine and are therefore more likely to stay on treatment long term.

This may be particularly important for people who have already reached their target weight. In these situations, maintaining weight loss with a treatment that feels easier to continue long term may be more important than achieving the maximum possible weight loss. Dietary changes and regular exercise are also essential for successful long term weight loss.
