How to overcome a weight loss plateau on GLP-1 treatment

If you’ve been using a GLP-1 medicine like Wegovy (semaglutide) or Mounjaro (tirzepatide) and your weight loss has slowed or stopped, you’re not alone. It can feel frustrating, especially if you’re still eating well, taking your medicine as prescribed, and trying your best.

Weight loss plateaus are common. In fact, they’re expected. This article explains why they happen, what you can do about them, and how to tell if it’s time to adjust your plan.

What is a weight loss plateau?

A plateau means your weight has stayed about the same for several weeks, despite using treatment and making healthy lifestyle choices. It’s a normal part of weight loss and not a sign that you’re doing something wrong.

In clinical trials, even people taking GLP-1 medicines under close supervision hit plateaus. In the SURMOUNT-1 trial, participants taking tirzepatide lost the most weight in the first 36-40 weeks. After that, weight loss slowed down. (1)

The same was true in the STEP trials for semaglutide. People who kept using the medicine still saw gradual progress, but it was slower and less steady than at the start. (2)

This is how the body responds to change. As your weight decreases, your body adapts. You may need fewer calories than before, and your body might start to resist further weight loss.

Common reasons for a plateau

Some plateaus are expected. Others may be linked to practical or lifestyle factors, including:

  • You’re eating more without realising it (e.g. grazing or larger portions)
  • Your activity levels have changed or dropped
  • You’re not drinking enough water or getting enough sleep
  • You’re eating less, but also moving less, which means your overall energy use is lower
  • You’re not in the same routine you had at the start of treatment

Sometimes people get stuck on a lower dose for longer than expected, or their current dose is no longer having the same effect. But this isn’t always the case, and higher doses don’t always mean more weight loss.

What you can do

1. Reflect gently and honestly

Think about what has changed. Has your portion size crept up? Are you moving less than you were before? Has a busy or stressful period made it harder to stay in your routine?

This isn’t about blame - it’s about understanding what your body might be responding to.

2. Revisit what worked at the start

Think back to when you felt most in control of your eating. What helped? Was it regular mealtimes? Drinking more water? Planning meals in advance?

GLP-1s work best alongside the habits that support them. If some of those have slipped, that could explain the plateau.

3. Stay curious about your hunger cues

GLP-1s usually make you feel fuller faster. But over time, you may notice changes, especially if your lifestyle, hormones or routine have shifted. Notice when you feel true hunger, when you’re satisfied, and whether your cues are still reliable.

4. Don’t rush to change your dose

It can be tempting to jump to a higher dose, but that’s not always the answer. We will look at the bigger picture - including how long you’ve been on your current dose, how your body is responding, and whether other factors could be affecting your results.

If a dose change is appropriate, it will be made safely and gradually.

When to ask for help

You don’t need to wait until you’re stuck or struggling to reach out. The prescribing doctor or pharmacy team can help you understand what’s normal, what might need reviewing, and what to expect next.

It might be time to check in if:

  • You’ve been on the same dose for several months with no change
  • You’re feeling constantly hungry or not getting full from meals
  • You’re unsure if you’re still in a calorie deficit
  • You’ve regained weight and feel unsure what to do next

GLP-1 treatment isn’t just about numbers on the scale. It’s about long-term support, sustainable changes, and staying connected to your care team when things get tough.
