New to all weight loss medications

If you are new to all weight loss medications or have been previously taking Mysimba, Ali or Orlistat-based weight loss medication, we recommend starting the 1.5mg of Wegovy pill. The manufacturer Novo Nordisk recommends a specific Wegovy pill dosing guide, starting on the lowest dose, because your body will need time to adjust to a new medication to avoid harsh side effects (1).

What is the Wegovy pill?

You’re probably thinking, what is the Wegovy pill and how is it different from injections? The Wegovy pill is a once-daily weight loss tablet for adults who are overweight or obese. It could be a useful option for people who don't like needles, have struggled with injection pens, have experienced adverse events at injection sites, or would simply prefer a tablet.

The Wegovy pill contains Semaglutide, a GLP-1 receptor agonist that helps:

Reduce appetite
Increase feelings of fullness
Slow stomach emptying
Support long-term weight management

While the Wegovy pill is new as a weight management treatment, the active ingredient semaglutide isn't. It's the same active ingredient already used in Wegovy injections.

Unlike traditional weight loss tablets, it works on the same biological pathways as injectable GLP-1 medicines such as Wegovy injections and Mounjaro. The main difference is that instead of a weekly injection, Semaglutide is taken as a daily tablet in the morning.

The Wegovy pill comes in several strengths, with treatment starting at the lowest dose and gradually increasing before reaching the 25mg maintenance dose.

Like all GLP-1 treatments, the Wegovy pill will work best alongside healthy lifestyle changes and will need to be taken correctly to achieve the best possible results.

What’s the difference between the Wegovy pill and injection?

Although both the Wegovy pill and injection contain Semaglutide and work in the same way, if you are deciding between Wegovy injections and the Wegovy pill, there are a few practical differences between them:

Who is the Wegovy pill suitable for?

The Wegovy pill may be particularly appealing if you:

Wegovy pills, like Wegovy injections, are a prescription-only weight management treatment, and suitability must be assessed by a clinician through an online consultation before it can be prescribed.

The criterion is, patients need to:

Have a BMI of 30 or above, or
Have a BMI of 27 or above alongside a weight-related health condition such as high blood pressure, high cholesterol, or type 2 diabetes

Meeting the BMI criteria alone does not automatically mean Semaglutide will be right for you. Before prescribing the Wegovy pill, your clinician will consider factors like your medical history, current medications, and previous side effects or reactions to determine whether it’s safe and appropriate for you.

Who should not take the Wegovy pill?

Wegovy pill is not suitable for everyone and should not be used:

During pregnancy or breastfeeding
Alongside other GLP-1 medicines such as Mounjaro or Wegovy injections
By children or adolescents under 18
By people who have previously had an allergic reaction to Semaglutide

The suitability of the Wegovy pill will also be dependent on any other medication you take or conditions you have been diagnosed with; this will be assessed by a clinician before being safely prescribed.

Will I get the same weight loss as the Wegovy injection?

The 25mg Wegovy pill is considered bio-equivalent to the standard Wegovy 2.4mg injection dose, meaning it may provide similar Semaglutide exposure and similar weight loss results (1,2).

In clinical trials, people taking oral Semaglutide 25mg lost an average of 16.6%* of their starting weight over 71 weeks. Among those who took the treatment exactly as prescribed, average weight loss was around 16.6%, which is similar to the results seen with Wegovy 2.4mg injections (1-2).

However, Wegovy injections are also available in a higher 7.2mg maintenance dose for some people who need additional support. This higher dose has been associated with greater average weight loss (19-21%) when compared to the Wegovy pill or the 2.4mg injection (3).

This means:

Wegovy 25mg pill ≈ Wegovy 2.4mg injection
Wegovy 7.2mg injection may produce greater weight loss

Weight loss varies from person to person, and taking the Wegovy pill exactly as instructed is important to help your body absorb the medicine properly.

Advisory information

For menopausal women taking HRT, you should consider changing oral oestrogen to transdermal oestrogen and also increasing the dose of progesterone for 4 weeks after initiation of a GLP-1 or after any dose change. You should discuss this further with your GP or prescribing Doctor.

How to use Wegovy Weight Loss Pills

Swallowing a tablet may sound simple, but with the Wegovy pill, it needs to be taken correctly for the treatment to work to its full effect. To help the pill work properly, you should:

Take it once daily, first thing in the morning
Take it on an empty stomach after fasting overnight (at least 8 hours after last meal)
Take it with no more than 120ml of plain water
Swallow it whole without splitting, crushing, or chewing the pill
Wait at least 30 minutes before eating or drinking anything other than water
Wait at least 30 minutes before taking any other oral medicines

If you miss a dose, skip it and take your next tablet at the usual time the following day. Do not take two doses together to make up for a missed tablet.

How does Wegovy pill dose escalation work?

To help your body adjust to treatment and reduce side effects, the Wegovy pill is started at a low dose and increased gradually over time. The expected dosing schedule is:

Weeks 1–4: 1.5mg once daily
Weeks 5–8: 4mg once daily
Weeks 9–12: 9mg once daily
Week 13 onwards: 25mg once daily maintenance dose

You may stay at a dose longer if your side effects don’t settle down. Always take the strength prescribed by your clinician and don’t rush through the steps. Taking a higher dose than recommended will not speed up weight loss and may increase the risk of side effects.

Can I take other medicines alongside the Wegovy pill?

You can take most other medicines alongside the Wegovy pill, as long as oral medicines are taken at least 30 minutes afterwards. But as Semaglutide slows stomach emptying and can cause side effects such as vomiting or diarrhoea, it can change how well some are absorbed.

This may be particularly important for:

Warfarin and other oral anticoagulants, where extra blood test monitoring may be needed to check that your blood clotting medicine is still working as it should.
Levothyroxine used for thyroid disease, which your doctor may need to check your thyroid levels if you are taking Wegovy together with levothyroxine.
Oral contraceptive pills, which may become less effective if vomiting occurs within 3–4 hours of taking them. Extra contraception is recommended until digestive side effects calm.

The Wegovy pill also shouldn’t be taken alongside any other GLP-1 receptor agonists or prescription weight loss drugs, such as Mounjaro or Wegovy injections.

Always tell your clinician about any medicines you’re taking before starting treatment.

Side effects of Wegovy Weight Loss Pills

The most common side effects with the Wegovy pill are digestive, usually most noticeable when starting treatment or increasing the dose. In clinical trials, around 7 in 10 people experienced at least one gastrointestinal side effect (1).

The most common included:

Nausea
Diarrhoea
Vomiting
Constipation
Stomach pain
Bloating or indigestion

These side effects are usually mild to moderate and often improve as your body adjusts to treatment.

Some people may also experience dizziness, tiredness, and temporary hair shedding during rapid weight loss.

Are there any serious side effects of the Wegovy pill?

Serious side effects with the Wegovy pill are uncommon, but you should stop treatment and seek urgent medical support if you develop:

Severe or persistent stomach pain, with or without nausea, vomiting, and/or fever
Swelling of the face, lips, tongue, or throat
Difficulty breathing, wheezing, or a fast heartbeat
Sudden or worsening changes to your eyesight, especially if you have diabetes
Severe constipation with bloating, stomach pain, or vomiting

These symptoms may be signs of pancreatitis, severe allergic reactions, vision complications, or bowel obstruction — all serious side effects that need urgent attention.
