Stopping Wegovy Pills: Do you have to take weight loss tablets forever?

One of the most common questions people ask about new weight loss tablets like the Wegovy pill is whether they'll need to take them forever.

Stopping treatment is always an option, but it's important to understand how it may affect your weight loss results. GLP-1 pills, like the Wegovy pill, are designed to support long-term weight management, and stopping can lead to some weight regain. Many people continue treatment after reaching their target weight to maintain their results, while others are able to maintain some weight loss without medication.

Do you have to take Wegovy tablets forever?

There is no requirement to stay on any weight loss tablets permanently, and you can choose to stop treatment at any time. Some people stop because they aren’t getting the results they hoped for, find the Wegovy pill side effects difficult to manage, or don’t have the financial means to continue.

Other people choose to stop once they reach their goal weight. There is nothing inherently wrong with this, but before you make that decision, it’s important to understand that stopping treatment means you will lose the effects that helped you lose weight in the first place.

As these wear off, many people find they start putting weight back on. For that reason, the Wegovy tablet should be seen as a long-term treatment, not a short-term solution.

What happens when you stop Wegovy pills?

GLP-1 medications like the Wegovy pill help you eat less without constantly feeling hungry, so that you gradually lose weight. They do this by reducing appetite, keeping food in the stomach for longer, and helping you feel fuller after eating (1).

But when you stop taking them, these effects on appetite slowly wear off. You may notice that:

  • You feel hungry more quickly between means

  • Food noise and cravings become stronger

  • Smaller portions no longer feel as satisfying

  • You start eating bigger meals or snacking more

These changes don’t usually happen overnight because the medicine doesn’t immediately leave your body. Semaglutide, the active ingredient in the Wegovy tablet, remains in the body for up to 7 weeks after taking your last tablet. As the medication is gradually eliminated, its appetite-suppressing effects fade too.

Without that support, many people find it harder to maintain the eating habits that helped them lose weight. Over time, this can lead to weight regain.

Will you regain weight after stopping the Wegovy pill?

Everyone’s experience is different, but many people do regain at least some of the weight they lost after stopping weight management medication.

The best evidence for this comes from studies on Semaglutide, the active ingredient in Wegovy tablets and Wegovy injections. In the STEP-4 extension trial, all participants used Wegovy injections for 20 weeks. Half of the participants were then put on a placebo, while the others remained on treatment for a year.

Over that year, those who stayed on treatment continued to lose weight. But those who stopped regained two-thirds of the weight they had lost on average (2). A recent review and meta-analysis of 37 studies involving more than 9,000 participants had similar findings: people regained an average of 0.4kg per month after stopping treatment (3).

This doesn’t mean you will definitely regain two-thirds of the weight you’ve lost or 0.4kg per month. Those figures are just trial averages, and individual responses vary. But according to evidence, weight regain is common when stopping GLP-1 weight loss medications (2).

Can you keep weight off without continuing weight loss treatment?

It is possible to keep weight off after stopping the Wegovy tablet, but it takes a conscious and consistent effort. Studies consistently show that at least some weight regain is common after stopping treatment (2-3).

This doesn’t mean all of the weight you lose will necessarily come back. In the STEP 4 trial, participants who stopped Semaglutide still maintained clinically meaningful weight loss by the end of the study. On average, they remained around 5% lighter than they were before treatment began (2).

But, the study only followed participants for 48 weeks after treatment was stopped. It's therefore difficult to know whether that remaining weight loss would have been maintained long-term or whether weight regain would have continued over time.

This is why patients are encouraged to focus on sustainable habits alongside treatment that they should then continue after stopping weight loss treatment, such as:

For many people, however, continuing treatment provides the best chance of maintaining their results.

Is long-term GLP-1 weight loss treatment safe?

Current evidence suggests that long-term GLP-1 treatment is generally safe for most patients.

The active ingredient in the Wegovy tablet, Semaglutide, has been studied for several years and is already used long-term by many patients worldwide.

As with any medication, regular reviews are important to ensure the benefits continue to outweigh any risks. If treatment is working well and side effects are manageable, many people can continue taking GLP-1 weight loss medication safely for extended periods under medical supervision.

Is stopping the Wegovy pill a bad idea?

Not necessarily. While weight regain is common after stopping GLP-1 treatment, that doesn't mean continuing treatment forever is the right choice for everyone.

The key is understanding the trade-offs. Stopping treatment means losing the effects of the tablets, which makes maintaining weight loss more difficult for most people. However, some people are still able to maintain meaningful weight loss without ongoing medication.
