How to take the Wegovy pill correctly (and why it matters)

The Wegovy pill offers a needle-free alternative to Wegovy injections, and many people may prefer the idea of taking a daily tablet. But it’s not as simple as swallowing it whenever it's convenient. It needs to be taken once daily on an empty stomach and carefully timed around food and drink.

Taking the pill as instructed helps it work properly. The pill contains Semaglutide, which is naturally poorly absorbed when taken orally. To help the medicine get into your bloodstream, nothing else can be inside your stomach: no food, other medicines, and very little water.

That’s why the Wegovy tablet has such strict restrictions around food and drink. In this guide, we explain exactly how to take it correctly, so that the medicine works as it’s supposed to.

How should you take the Wegovy tablet?

To be absorbed properly, the Wegovy tablet should be taken according to these instructions (1):

  1. Take one tablet daily in the morning, ideally at the same time each day

  2. Don’t crush, chew or split the tablet - it should be swallowed whole

  3. The tablet needs to be taken on an empty stomach after overnight fasting

  4. Swallow the tablet using a small amount of plain water (up to 120ml)

  5. Wait at least 30 minutes after taking the Wegovy tablet before eating, drinking (even more plain water), or taking other medications*

  6. Remain upright (sitting or standing) during this 30-minute window to facilitate absorption

*If you are taking Levothyroxine and Wegovy tablets, dosing will vary slightly. We recommend taking the Wegovy tablet first upon waking with 120ml of water, wait at least 30 minutes, then take levothyroxine, and wait an additional 30-60 minutes before eating. Alternatively, consider taking your levothyroxine dose consistently at bedtime (3-4 hours after the last meal).

What does this look like in practice?

Although the instructions may seem unusual at first, they soon become part of a simple morning routine. Here is what this routine could look like for many people:

Before you take the Wegovy pill

The Wegovy tablet needs to be taken on an empty stomach, which is why it’s recommended to take it first thing in the morning after waking up, before you've had anything to eat or drink.

If you’re a late-night snacker or typically wake up during the night to eat, try to avoid doing so. Any food you eat during the night may still be in your stomach in the morning. Taking the tablet after an overnight fast gives your body the best chance of absorbing the medicine properly.

So that you remember to take the Wegovy pill, it can help to prepare the night before. You may:

  • Set a daily reminder on your phone until taking the Wegovy pill becomes a habit.

  • Keep the tablet and a small glass of water beside your bed so that it’s there ready for you when you wake up.

These small steps may be especially useful if you’re usually rushing out the door and can help prevent you from forgetting or missing a dose.

Taking the Wegovy pill

Although it can be tempting to take your tablet with your morning coffee or another drink, it should only be taken with plain water. Here are the steps to follow:

  1. Wake up and take the Wegovy tablet before food, drink, or other medicines

  2. Swallow the pill whole with no more than 120ml of plain water

  3. Set a timer for 30 minutes on your phone

Setting a timer isn't essential, but many people find it helpful. It's easy to lose track of time while getting ready in the morning and accidentally eat, drink or take other medicines too soon. Waiting the full 30 minutes gives the tablet the best chance of being absorbed so that it can work.

After taking the Wegovy pill

During the next 30 minutes, you need to avoid eating, drinking anything (including more plain water), and taking other oral medications to make sure the medicine is absorbed properly.

If you’re used to waking up and having breakfast or a coffee right away, it may take a few days to adjust to the new routine. Many people use the waiting period to shower, get dressed, or prepare for the day, then have breakfast, coffee, and other medications once the 30 minutes have passed.

Why does oral Semaglutide have such strict instructions?

The strict instructions might seem like a nuisance, but they’re there to help make sure the medicine makes it into your bloodstream so that it can work as intended.

For any oral medication to work, it needs to pass through the stomach lining and into the blood. Semaglutide is naturally very poorly absorbed when taken as a tablet. If left to fend for itself in the stomach, much of it would be broken down before it ever reached the bloodstream.

To overcome this problem, the Wegovy tablet contains an ingredient called sodium N-(8-[2-hydroxybenzoyl] amino) caprylate, or SNAC for short. SNAC helps Semaglutide survive long enough in the stomach to be absorbed, but it only works under very specific conditions.

Food, drinks, large amounts of water, and other medications can interfere with the process. This is why the Wegovy tablet must be taken on an empty stomach, with a small amount of water, and followed by a 30-minute waiting period.

If not, less of the medicine may be absorbed, and the medicine might not work as well as it could.

How does SNAC help Wegovy tablet absorption?

As the Wegovy tablet dissolves, SNAC helps Semaglutide get into the bloodstream in two ways (2):

  1. Protecting it from being broken down too quickly in the stomach.

  2. Helping it cross the stomach lining and enter the bloodstream.

These changes only occur for a short period and in a small area around the tablet. Anything else entering the stomach during this process, including food, drinks, other medications, or even large amounts of plain water, can affect how well SNAC does its job.

This is also why you should never crush, chew, or split the Wegovy tablet. The tablet has been carefully designed so that Semaglutide and SNAC are released together as it dissolves. Breaking the tablet apart could disrupt this process and cause less of the medicine to enter the bloodstream.

How important is it to follow the Wegovy pill instructions correctly?

The instructions really do matter because of how inefficiently oral Semaglutide is absorbed.

For context, people switching between Wegovy tablets and injections may move directly between the 25mg tablet and the 2.4mg injection, even though the tablet contains more than ten times as much Semaglutide.

The tablet has to contain a much larger amount of the medicine because absorption through the stomach is naturally difficult, even with the help of SNAC. The administration instructions are designed to give the medicine the best possible chance of being absorbed.

What if you forget the Wegovy pill in the morning?

If you forget to take the Wegovy tablet before breakfast, the best option is to skip the forgotten tablet and take your next dose at the usual time the following morning.

While some Semaglutide may still be absorbed if you take the tablet later in the day after eating and drinking, absorption is much less predictable, so it’s better to wait. When taking your next dose, do not take two weight loss tablets to make up for a missed one.

If you simply woke up later than usual or forgot to take your tablet right away but haven’t yet eaten or drunk anything, you can go ahead and take your tablet as soon as possible. You will need to have your breakfast a little later to allow for the 30-minute window after swallowing the pill to pass.

Before you worry, missing the occasional dose is unlikely to ruin your progress. But do try to get into a routine, as taking the tablet consistently gives you the best chance of achieving long-term weight loss.
