Starting a GLP-1: How it might feel (and why that’s normal)

It’s okay if this doesn’t feel how you expected

You’ve made the big decision to get healthier with GLP-1 injections. Many people expect to feel excited to start treatment and proud that they’ve made a step toward improving their health. But if you instead feel uncertain and nervous about what happens next, that’s totally normal. 

Starting GLP-1 treatment is often more emotional than people expect. Our weight is extremely personal, and the journey can look very different from what you imagined. Importantly, there is no “right” way to feel, and feeling unsure or disheartened doesn’t mean you’re doing anything wrong. It’s all part of the process. 

Let’s be honest: This isn’t magic, and that’s a good thing

There is a huge hype around GLP-1 medicines, and many myths are flying around. You may have read stories in the media of people who claim to feel like a “new person” overnight, who feel full immediately, and whose weight suddenly dropped off. 

But GLP-1s aren’t magic. The reality is that:

  • Changes often happen slowly and subtly
  • You will still feel hungry at times
  • You may feel nothing in the first few weeks

All this is okay and completely normal. GLP-1s aren’t designed to flip a switch, instantly turn off hunger cues, and cause rapid weight loss. They reduce appetite to support gradual, sustainable, healthy change (1). You don’t need to rush to feel something dramatic. And if at all unsure, support is part of the process. What people often feel in the first few weeks

The first month of starting GLP-1s is the most challenging for many. Everything is new, and people often don’t know what to expect. While everyone’s experience is very different, some of the phrases we hear again and again in the early stages are:

  • “I thought I’d feel different by now”
  • “I still feel hungry sometimes – is that bad?”
  • “I’m scared I’ll lose control again”
  • “This doesn’t feel like enough… should I be doing more?”
  • “I feel a bit disconnected from myself”

It’s also common to grieve old coping patterns, such as eating for comfort, reward, or routine. You might feel low or disoriented because these old habits, despite being unhelpful and unhealthy, perhaps provided structure, distraction, or short-term comfort. 

If any of these thoughts sound familiar, you’re not alone. Emotional wobbles and uncertainty are part of the adjustment period. Your brain and body are learning something new, and it can take the first month to find your rhythm, or perhaps even longer. 

Remember, GLP-1s are a long-term investment. Many people give up on treatment before they see real results (2). Be patient with yourself and try to trust the process.

Shame, guilt, and second-guessing are part of this too

Some people are proud to start weight management treatment, pleased that they’re taking an active role in their health. Others feel deep shame and guilt. There is a common yet unspoken belief that you should have been able to manage without help, and needing medication means you’ve failed.

This belief is wrong. If you’re feeling shame or doubt creep in, know that: 

  • Weight isn’t about willpower. Weight is shaped by biology, stress, sleep, hormones, and so much more. Willpower might play a small role, but it’s never the full story.
  • Needing support isn’t a weakness. It’s a sign that you’re taking your health seriously. Asking for help with your weight shows strength, not failure. 
  • Medication isn’t a shortcut or a cop-out. Just like using an inhaler for asthma or insulin for diabetes, using GLP-1s is a way to support your body, not avoid responsibility.

How to be gentle with yourself

When starting GLP-1s, the best thing you can do is go slow and listen to your body. Eat when you’re hungry. Rest if you’re tired. Ask questions if you’re unsure. 

It’s also important to turn your attention away from the scales. Yes, weight loss is the goal of GLP-1s, and you’ll hopefully see slow and sustainable weight loss with time. But, depending on your starting weight, it can take a year of treatment until you see full results (3). While some people may lose weight within the first month, early progress doesn’t always look like shrinking numbers. 

Progress might be thinking less about food, feeling more in control of your appetite, or even sleeping better, having more energy, and walking more easily. These are all real wins. While not as black and white as a number on a scale, each shows genuine improvements in your overall health.

This is care, not punishment

If you’re struggling with the mix of emotions of starting GLP-1s, remember that you’re doing something kind for yourself. You’re caring for your body. You’re making steps towards a healthier future. And you’ve already started your journey, which is something to celebrate.

Your experience might be difficult at first and far from what you expected. But don’t second-guess yourself too much or worry about rigid rules and restrictions. It’s not about punishing your body into change but working with it for slow, steady, and sustainable weight loss.
