How to take Foundayo (orforglipron)

Foundayo boxes 0.8mg, 2.5mg, 5.5mg, 9mg, 14.5mg and 17.2mg strengths

For many an oral GLP-1 weight loss medication like Foundayo (orforglipron) is preferred to self-administered injections. This once-daily oral treatment is prescribed alongside a reduced-calorie diet and increased physical activity to help adults living with obesity to lose excess weight. However, when and how you take Foundayo is very important for it to be effective (1).

What dose of Foundayo (orforglipron) is right for me?

You will start Foundayo on the lowest dose. This is to help reduce the risk of Foundayo side effects. Like other GLP-1 medications, Foundayo supports weight loss by changing your appetite and slowing down your digestion. While this means that you will usually eat less, digestive side effects such as nausea, vomiting, diarrhoea and constipation are common.

Starting on a lower dose and gradually increase (titrating) gives your body time to adapt to the medication, making treatment more comfortable and improving the likelihood that you will be able to stay consistent long-term (2).

It is important to remember that your weight loss journey is personal to you, and while there is a recommended dose titration (increase) schedule, you do not have to follow this exactly, especially if it seems to be taking your body longer to get used to the medication. There is nothing to say that you cannot stay on a lower dose of Foundayo longer term if it is providing satisfactory weight loss and you are able to manage your side effects well (3).

What is the Foundayo (orforglipron) dosing schedule?

The UK dosing schedule is now confirmed in Foundayo’s Patient Information Leaflet (PIL) and follows a spaced titration schedule of 6 doses at least 30 days apart (4).

Week

Dose

Should I step up my dose?

1 to 4

0.8mg once daily for 30 days

This is your starting dose, and you will stay on this for at least 30 days.

5 to 8

2.5mg once daily for 30 days

If you are tolerating 0.8mg well, you should step up to the 2.5mg dose. If you are losing weight consistently, you can stay at this dose or choose to increase it. However, it is important to note that your side effects may increase each time you step up your dose.

9 to 12

5.5mg once daily for 30 days

If you are losing weight consistently at this dose, you can choose to stay on it or increase to the next.

13 to 16

9mg once daily for 30 days

Still losing weight consistently? You may want to stay here, or you could move up again.

17 to 20

14.5mg once daily for 30 days

You are nearing the end of the titration ladder, but you could step up to the next dose if you’re not losing weight consistently.

21+

17.2mg once daily for 30 days

17.2mg is the final maintenance dose. You cannot increase beyond this.

Beyond 2.5mg, further increases are based on treatment response and your ability to tolerate side effects, rather than a fixed timeline. There should be a minimum of 30 days between each dose escalation (5).

Do side effects get worse when you increase your Foundayo dose?

Mild digestive side effects are common when taking any GLP-1 medication, and Foundayo is no exception. In clinical trials, 60-69% of people experienced them, depending on which strength tablet they were taking (6).

Typically, side effects should get better as your body gets used to treatment, but if things are not settling down and you are still finding them hard to manage you may want to consider:

  • Delaying your next dose increase

  • Staying on your current dose as maintenance

  • Reducing your dose temporarily

Slowing things down can help make it easier to stick to your treatment plan long-term.

What time of day is best to take Foundayo (orforglipron)?

There is not an official best time of day to take Foundayo. The Patient Information Leaflet states that you can take it at any time of day, with or without food. This means that you can take it at the time of day that is most convenient, and to help make it a consistent habit, most convenient for you (5).

How to take Foundayo (orforglipron)

Foundayo should be taken once daily, swallowed whole. You should not break, crush or chew the tablet. You can take the tablet with or without food, and with as much water as you need (5).

Taking Foundayo once every day helps you stay on the treatment schedule used in clinical trials. The trials did not directly measure whether people who miss occasional doses have worse weight-loss results, but regular daily use is important because missing a week or more may require your prescriber to restart dose escalation at a lower dose (4,5).

Should I take Foundayo (orforglipron) with water?

Yes, you can take water to help you swallow your Foundayo tablet (5).

Should I take Foundayo (orforglipron) with food?

You can take Foundayo with or without food, the choice is yours (5).

If there is one thing to remember about Foundayo dosing, it is that the gradual approach is there to support you, not slow you down. Allowing your body time to adjust to each dose can help make treatment more sustainable and effective in the long term. Just remember:

  • 30 days is the minimum time spent on each dose, not a target.

  • The highest dose is not necessarily the best dose for you.

  • Successful treatment is about finding a balance between weight loss and side effects.

  • For weight loss to be most effective, you should combine treatment with a balanced diet and regular exercise.
