Foundayo starts at a low dose to help reduce the risk of Foundayo side effects.
Like other GLP-1 medications, Foundayo supports weight loss by changing your appetite and slowing down digestion. While these effects help you eat less, they can also cause digestive side effects that tend to settle once your body gets used to the medication.
Common side effects include (1):
Nausea
Diarrhoea
Constipation
Vomiting
Starting on a lower dose and increasing it gradually gives your body time to adapt, making treatment more comfortable and improving the likelihood that you'll be able to continue long-term.
What is the dose escalation schedule for Foundayo?
The UK dosing schedule is now confirmed in Foundayo's Patient Information Leaflet, and it follows a spaced titration schedule of 6 doses spaced at least 30 days apart (3).
The dosing schedule for Foundayo is as follows (3):

Beyond 2.5mg, further increases to 5.5mg, 9mg, 14.5mg, or 17.2mg are based on treatment response and tolerability rather than a fixed timeline, so not everyone follows every step of this schedule (3).
When do you reach the top maintenance dose of Foundayo?
If you follow the dosing schedule exactly, moving to a stronger dose every 30 days until reaching the top maintenance dose of 17.2mg, it will take around six months to reach the highest available strength of Foundayo.
But not everyone needs to follow the full schedule.
Once you reach 2.5mg, your prescriber may recommend staying there as your maintenance dose if you're losing weight successfully, have good control over your appetite, and are tolerating treatment well (3). Others may continue to 5mg, 9mg or 14.5mg and stay there, or continue further to 17.2mg (3).
It's also worth remembering that 30 days is the minimum time spent on each dose. If you need longer to adjust or are experiencing side effects, your prescriber may recommend delaying the next increase, in which case the titration schedule will take longer to complete.
The goal isn't to reach the highest dose as quickly as possible. It's to find the dose that helps you lose weight comfortably, and to do so at a rate that works for you.
What does each Foundayo dose increase feel like?
When first starting a GLP-1 medication like Foundayo, you’ll probably notice some digestive side effects, but you might still feel hungry and might not see much, if any, weight loss. This is completely normal.
During the first month of treatment, the priority is helping your body adjust to Foundayo rather than losing weight. Some people do notice changes in hunger and fullness in the first month, while others don’t notice appetite suppression until they reach higher doses, and that’s okay.
As your dose increases, you may notice:
Feeling full after eating smaller portions
Reduced hunger between meals
Fewer food cravings
Finding it easier to stick to a calorie deficit
More consistent weight loss
It's also common for digestive side effects to temporarily return with each dose increase. This doesn't necessarily mean the dose is too high or that treatment isn't right for you. In many cases, these symptoms will improve as your body adjusts to the new, higher dose of Foundayo.
Will you experience side effects when increasing your Foundayo dose?
Mild digestive side effects should be expected at all doses increases. In clinical trials, 60 to 69% of people experienced digestive symptoms, depending on which strength tablet they were taking (4).
But they typically improve as your body adjusts to the treatment. If yours don’t seem to be settling down or you're finding them difficult to manage, talk to your prescriber. They may recommend:
Delaying your next dose increase
Staying on your current dose as maintenance
Reducing your dose temporarily
That is one of the great things about the Orforglipron titration schedule: once you’ve increased to 2.5mg, you can choose to stay on 2.5mg or increase to the following doses. Put simply, past 2.5mg you can either stay on dose or increase to reach 17.2mg as long as you’ve been on each step for 30 days. Slowing things down can make treatment easier to stick with if side effects are bad, so try not to see taking longer to reach maintenance as a bad thing.

Can I reduce my Foundayo dose or stay at a certain dose of Foundayo?
Yes. While some people follow the dose-escalation schedule up to the maximum, treatment can be adjusted based on how well you tolerate each dose. Many people pause at lower maintenance doses (5.5mg, 9mg, 14.5mg) or move back down if side effects become difficult to manage after increasing tablet strength.
Any dose changes should be made under the guidance of your prescriber rather than independently.
How does Foundayo dosing compare to the Wegovy tablet?
Both Foundayo and the Wegovy Pill use gradual dose escalation with a standard schedule of 30 days on each strength before increasing. The main difference is the number of steps:
Wegovy tablets come in four strengths, meaning most people reach the recommended maintenance dose (25mg Semaglutide) after around four months (5).
Foundayo comes in six strengths, so it can take around six months to reach the highest maintenance dose available (17.2mg Orforglipron) (3).
But the longer titration schedule also gives Foundayo more flexibility. While Wegovy has one recommended maintenance dose, Foundayo offers several potential maintenance doses depending on your treatment experience.
There’s also another important distinction between the Wegovy pill and Foundayo: one is simpler to take. While the Wegovy tablet must be taken on an empty stomach at least 30 minutes before eating, drinking, or taking other medications, Foundayo can be taken at any time, with or without food (5).
This flexibility, both in maintenance dose options and how the tablet is taken, could make Foundayo a more convenient oral option for some people.
