How long does Mounjaro stay in your system and why does it matter?

If you are stopping Mounjaro injections because you’re planning a pregnancy, experiencing side effects that are difficult to manage, having a surgical procedure, or simply don’t want to continue with treatment, you probably want to know how long the medicine will stay in your system.

Although Mounjaro is taken weekly, the medicine won’t have left your body by the time your next weekly dose would have been due. It takes around 25 to 30 days after your last injection for most of the medicine to leave your system, though the effects of Mounjaro may fade faster (1,2).

Here, we’ll explain in more detail how Mounjaro builds up and leaves your system and why this matters if you plan on stopping or pausing treatment.

How long does Mounjaro actually stay in your system?

Most Mounjaro will have left your body around 25 to 30 days after your last injection (1-3).

The amount of Mounjaro gradually falls each day after your last dose as your body breaks down and removes Tirzepatide, the active ingredient in Mounjaro injections (1). As Tirzepatide levels fall, its effects on appetite and hunger will gradually fade too.

There can be some variation between people, so there isn't an exact day when Mounjaro suddenly disappears from your system. The timeframe of 25 to 30 days is an estimate based on Mounjaro's “half-life” – the time it takes for the amount of a medicine in your body to fall by half (1-3).

What is the Mounjaro half-life?

Mounjaro has a half-life of around 5 days (1,2). This means that around 5 days after an injection, half of the Tirzepatide from that dose remains in your body.

Scientists use the half-life of medications to calculate how quickly medicines are eliminated from the body. After around 4 to 5 half-lives, approximately 94% to 97% of a medicine has been eliminated, and levels are typically low enough for it to be considered effectively eliminated (3).

Does a higher Mounjaro dose take longer to clear?

No, Mounjaro is broken down and eliminated at the same rate regardless of your dose (1,2).

Someone taking a higher Mounjaro dosage will have been exposed to more Tirzepatide than someone on a lower dose. But the body doesn’t clear a fixed amount of Mounjaro every day. Instead, the amount in your body falls by a similar proportion over time (2,3).

Whether you take the starting dose of Mounjaro 2.5mg, or Mounjaro 5mg, 7.5mg, 10mg, 12.5mg or 15mg, the half-life is approximately 5 days (1-3).

Does Mounjaro build up in your system?

Mounjaro gradually builds up in your system during the first few weeks of weekly injections (1,2).

Because Mounjaro stays in your system for around a month, there is still some in the body when it’s time for your next injection. Each new dose is added on top of what is left from your last one. But Mounjaro levels don’t keep building up with every injection.

Because your body removes a similar proportion of the Tirzepatide in your system, the more that has built up, the more that is removed between doses (2,3). After around 4 weeks, the amount of Tirzepatide added through your weekly injections is balanced by what your body removes (1).

This is known as reaching a “steady state” (1). Mounjaro levels still rise after each weekly injection and fall before the next one, but within a relatively stable range, even if you occasionally take Mounjaro a day early (1,2).

When does Mounjaro peak in your system?

Mounjaro levels peak in the blood around 8 to 72 hours after an injection (1). Many people feel the strongest effects on appetite around this time too.

Because Mounjaro builds up in your system with every injection for the first few weeks, each post-injection peak will be slightly higher than the last until you reach “steady state.” Once here, the peak level of Mounjaro will be roughly the same each week (1,2). If your Mounjaro dose is increased, your body will reach a new steady state at the higher dose.

The highest levels of Mounjaro in your system throughout your entire treatment plan will generally occur once you have reached steady state on the highest dose you take, during the 8 to 72 hours after an injection (1,2).

What happens when you stop taking Mounjaro?

If you stop Mounjaro weight loss injections, the amount of Tirzepatide slowly drops until most has been cleared around a month later (1-3). The effects Mounjaro has on “food noise”, appetite, and hunger will gradually fade too:

  • 1 week after your last injection: Around 38% of the Tirzepatide that was in your system when you stopped may remain, based on its half-life of 5 days. Your next Mounjaro injection would normally be due around now, so you may still notice its effects (1-3).

  • 2 weeks after your last injection: Around 14% may remain. Almost three half-lives have passed, and most of the Mounjaro that was in your system when you stopped has now been cleared. You may start noticing your appetite slowly starting to come back.

  • 3 weeks after your last injection: Around 5% may remain. Just over four half-lives have passed. As a general rule, medicines have usually fallen to levels too low to have much effect after around 4 to 5 half-lives (3). Your appetite may be similar to what it was before Mounjaro.

  • 4 weeks after your last injection: Only around 2% may remain. Almost six half-lives have passed, so Mounjaro has been largely cleared from your system (1-3). For most people, all effects on reducing appetite and hunger have worn off.

What impacts how quickly Mounjaro leaves your body?

There will naturally be some variation between people, but the same 25 to 30-day estimate applies to most people taking Mounjaro (1,2)

You might expect factors such as your age, body weight, sex, or how well your kidneys and liver work to significantly change how quickly Mounjaro leaves your body. However, these factors do not have a clinically relevant effect on how Tirzepatide is processed by the body (1,2).

How to get Mounjaro out of your system

There is no proven way to get Mounjaro out of your system faster. Once you have injected a dose, your body needs time to break down and remove the Tirzepatide naturally (1).

Drinking extra water, exercising, fasting or trying to ‘flush’ Mounjaro from your system will not make it leave your body faster. The only option is to wait. However, you don't need to wait until every last trace has left your body before its effects begin to wear off.

Why does it matter how long Mounjaro stays in your system?

Knowing that Mounjaro stays in your system for around a month after your last injection is important because stopping injections doesn't immediately stop the medicine from affecting your body.

There are several situations where this matters:

  • Planning a pregnancy: Mounjaro should not be used during pregnancy. If you are planning to become pregnant, you should stop taking Mounjaro at least one month beforehand to allow time for the medicine to leave your system (1).

  • Having surgery: Mounjaro slows the rate at which food leaves the stomach, which is relevant if you need a general anaesthetic because it can increase the risk of food entering the lungs. MHRA guidance says to tell your healthcare team that you take Mounjaro before having surgery, but never stop Mounjaro without first talking to your doctor (1,4).

  • Experiencing side effects: Mounjaro side effects may not disappear immediately after your last injection. If you are experiencing side effects, speak to your prescriber rather than assuming that stopping your next dose will make them disappear straight away (1).

  • Stopping or restarting treatment: If you stop Mounjaro for a period of time, the amount in your system will continue to fall. How long you have been without treatment may therefore affect how you restart Mounjaro. Speak to your prescriber rather than restarting at a previous dose after a longer break.
