Oxford Online Pharmacy use Onfido Limited to check that our patients are genuine - this is a regulatory requirement of the CQC and is performed once only when the first order is placed. Existing customers will be checked as they order again.

We ONLY ask Onfido to check three data sources for this, credit agencies, Voting register and Telephone Database.

The information provided by you (being the individual that signs this Declaration of Consent) will be validated by Onfido Limited (Company Number: 07479524) ("Onfido") and used for the purpose of identity verification and in the manner set out in this Declaration of Consent.

You have a right of access to your personal records held by credit reference and fraud prevention agencies and by Onfido. Your rights are set out in the Onfido Privacy Policy. Onfido may be contacted at: Onfido Ltd, 40 Long Acre, Covent Garden, London WC2E 9LG.

You agree as follows:

  1. I authorise Onfido and/or any of their appointed agents to carry out all necessary searches, including searches of consumer credit records, in order to verify my information.
  2. I have read Onfido’s Privacy Policy and I agree to all my personal information being processed in accordance with Onfido's Privacy Policy.
  3. I agree, to the maximum extent permitted by applicable law, that I will not hold Onfido and its officers, employees agents and/or customers liable for any direct, indirect, incidental, special, consequential or punitive damages, and to hold harmless Onfido, its officers, directors, employees, agents and customers from and against any and all claims, liabilities, damages, losses or expenses, including reasonable legal fees and costs, arising out of or in any way connected with use of Onfido's website (and other online properties) or any of the services provided by Onfido.
  4. I confirm to the best of my knowledge having exercised all due skill and care that the information I have given is complete, true and correct and I agree to the use of my information as detailed in this Declaration of Consent.
  5. If the ID check fails to pass using ONFIDO information then you may be asked to provide proof of identity which will be checked by our team. Once passed the information will be deleted and not kept on file.
