How long does Wegovy take to work?

If you’re starting Wegovy, it’s normal to wonder when you’ll notice changes in your appetite and weight. Wegovy starts working straight away but continues to develop over 4 to 12 weeks, with more consistent results around 4 to 6 months. Weight loss doesn't happen immediately.

If you're early in treatment and not seeing changes yet, that’s expected. This guide explains what typically happens week by week and what results you may realistically see from Wegovy.

When does Wegovy start working?

Wegovy begins working in your body soon after taking your first injection. It contains semaglutide, a medicine that mimics a hormone our bodies produce called GLP-1. GLP-1 regulates appetite by:

  • Slowing how quickly food leaves the stomach

  • Increasing feelings of fullness

  • Reducing hunger signals in the brain.

As soon as semaglutide enters your bloodstream, it starts activating the same pathways as natural GLP-1. But it takes time for these early changes inside the body to translate into noticeable appetite differences, reduced calorie intake, and steady weight loss.

What happens in the first few days of treatment?

After your first injection, semaglutide is slowly absorbed and reaches peak levels within 1 to 3 days. It stays active in the body for around a week, at which point you need to take your next dose (1).

Mild nausea is common in the first week, and some people notice early appetite changes, such as:

  • Feeling fuller sooner

  • Reduced “food noise

  • Less interest in snacking

However, everyone starting Wegovy begins on a 0.25mg dose. This is not a treatment-strength dose designed to cause weight loss, but rather to help your body adjust to the medication, so you may not feel much at all. That’s perfectly normal as well.

What happens during the first month on Wegovy?

The starting dose of 0.25mg is taken every week for the first month of treatment. As the weeks pass, the medicine builds up in the body, reaching a steadier level by weeks 4 to 5 (2). During this process, more people begin to notice subtle appetite changes.

Some people may also notice early weight loss, but this is often related to fluid changes rather than fat loss. It’s more typical for the number on the scale to stay the same. Both experiences are normal. Early progress is encouraging, but little change during the first month isn’t anything to worry about.

Remember, the starting dose of Wegovy is mainly about helping your body adapt to the medicine.

When does weight loss typically become noticeable?

For most people, weight loss becomes noticeable after 8 to 12 weeks of treatment. This is because Wegovy doses are increased gradually over time. As the dose rises, its effects on appetite typically become stronger, making it easier to maintain a calorie deficit.

A typical timeline for stepping up doses looks like this (3):

  • Weeks 1–4: 0.25mg weekly

  • Weeks 5–8: 0.5mg weekly

  • Weeks 9–12: 1mg weekly

  • Weeks 13–16: 1.7mg weekly

  • Week 17 onwards: 2.4mg weekly

Although some people lose weight earlier, 8 to 12 weeks is the midway point in the dosing schedule. While 1mg is not a target maintenance dose, it is an active dose that can cause real weight loss, and progress typically becomes easier to see.

Each dose step may briefly bring back side effects. Your prescriber may recommend staying on each step for longer than four weeks if side effects haven’t settled, so timelines can vary.

What happens after reaching your maintenance dose?

Many people continue increasing their dose until they reach the maintenance dose of 2.4mg at around 4 to 5 months. However, some people achieve good appetite control and weight loss at 1.7mg and may stay at this level, especially if higher doses cause side effects.

At maintenance dose, Wegovy produces its strongest consistent effects on appetite. Some hunger is perfectly normal, but you may notice:

  • More reliable appetite control

  • Fewer cravings between meals

  • Smaller portion sizes feeling satisfying

  • Steadier progress on the scale

By around 6 months of treatment, many people will have been at maintenance dose for several weeks. Clinical trials show average weight loss of around 11% to 14% of starting body weight by this stage, although individual results vary (4).

However, even after reaching the maintenance dose, weight loss is rarely linear. It’s normal to have faster and slower weeks, but overall weight loss should continue steadily. If it isn’t and you need further support, prescribers may consider increasing to a higher 7.2mg maintenance dose.

Why does Wegovy work faster for some people than others?

People respond to Wegovy at different speeds. Weight loss timelines can be influenced by (5):

  • Your starting weight

  • How quickly your dose increases

  • Eating habits

  • Physical activity levels

  • Whether you are male or female

  • Other health conditions

Some people notice appetite changes within the first few weeks, while others see clearer differences after reaching higher doses. What matters most is steady progress over time rather than rapid changes early on.

How much weight loss could you see long-term?

Weight loss with Wegovy continues gradually as you continue with your weight loss injections. In the STEP-1 trial, adults taking 2.4mg Wegovy lost an average of around 15% to 17% of their baseline body weight after 68 weeks (just over one year) (4). Within that group:

  • Around half lost at least 15%

  • Roughly one-third lost 20%+

People on the highest 7.2mg maintenance dose may see even more weight loss. Clinical trials show that at 68 weeks, participants lose an average of 19% to 21% of their starting weight (6).

While these results show what Wegovy can achieve, real-world results can vary. Some people lose less than the trial average, and some lose more, but steady weight loss over several months is the most common experience.

Does Wegovy work as quickly as Mounjaro?

Wegovy and Mounjaro both reduce appetite and support weight loss gradually over several months, but some people notice changes slightly earlier with Mounjaro.

This is because Mounjaro activates two appetite-regulating hormone pathways (GLP-1 and GIP), while Wegovy works through GLP-1 alone. This dual action has been linked to slightly greater average weight loss over time (7).

However, both treatments follow a similar dose schedule, and noticeable weight loss usually develops over 8 to 12 weeks rather than immediately.

What if Wegovy doesn’t seem to be working?

If your weight doesn’t change in the first month or two, it doesn’t usually mean Wegovy isn’t working. Because Wegovy works by reducing appetite and supporting a calorie deficit, progress normally becomes clearer at higher doses or once the maintenance dose is reached.

If you reach 2.4mg and aren’t losing much weight, we may consider increasing to the 7.2mg dose then our team can review this increase and other factors that may be limiting weight loss. Your prescriber may consider increasing to the 7.2mg dose, review other factors that could be limiting your progress, or suggest another weight loss treatment.

At Oxford Online Pharmacy, treatment includes ongoing clinical support as your dose increases, so your progress can be reviewed and adjusted if needed.
